Specialist Coordination & Navigation supports NDIS participants whose circumstances require a higher level of coordination across disability, mainstream and community systems.
At WIRU, Specialist Navigation is our sole NDIS service.
Our Specialist Navigators are qualified Social Workers who bring professional knowledge, systems experience and a collaborative approach to helping participants understand their NDIS plan, coordinate supports and navigate complexity.
Our goal is not simply to connect people with services. It is to help create a coordinated and sustainable support network around the participant while strengthening their understanding, choice and control.
When multiple services, professionals and systems are involved in a person's life, navigating them can become difficult.
WIRU works alongside participants and their support networks to help bring these different parts together.
Depending on the participant's individual circumstances, Specialist Navigation may include:
understanding and implementing the participant's NDIS plan
identifying appropriate NDIS, mainstream and community supports
coordinating services across multiple providers and systems
identifying gaps, barriers and risks within existing supports
strengthening communication between services and professionals
supporting complex transitions between services or living arrangements
assisting when support arrangements are at risk of breakdown
working collaboratively with guardians, nominees and other decision-makers
supporting participants to understand their options and make informed choices
building participant capacity and strengthening informal and community supports
monitoring how supports are working together
providing clear information and reporting relevant to the participant's NDIS goals and support needs
Throughout this work, the participant's goals, rights, preferences and choices remain central.
This is where Specialist Navigation matters.
Participants with complex circumstances may interact with several systems at the same time.
WIRU has experience navigating across:
NDIS & Disability Providers, behaviour support, accommodation pathways and complex support arrangements.
Health & Hospitals Hospital discharge planning, health services and coordination between health and disability supports.
Mental Health Community and forensic mental health services and psychosocial support systems.
Justice & Corrections Justice, correctional and forensic environments and transitions between custody and community supports.
Guardianship & Decision-Making Working collaboratively with guardians, nominees, advocates and other formally appointed decision-makers.
Housing & Accommodation Navigating housing systems and coordinating complex accommodation and support pathways.
Mainstream & Community Services Connecting and coordinating supports outside the NDIS where these form part of the participant's broader support network.
Specialist support for complex circumstances. WIRU works with NDIS participants whose circumstances may require experienced, professional navigation. This may include participants:
with complex or high-intensity support needs
living with psychosocial disability
involved with multiple services or government systems
transitioning from hospital, custody or other institutional environments
experiencing instability or breakdown within existing supports
navigating guardianship or formal decision-making arrangements
requiring coordination between NDIS and mainstream services
experiencing complex housing or accommodation circumstances
requiring multiple professionals and providers to work together
needing additional support to understand and implement their NDIS plan
Every participant is different. We take the time to understand the individual rather than defining people by a diagnosis, system or circumstance.
WIRU intentionally maintains small caseloads so our Navigators have the capacity to remain involved and understand the circumstances surrounding each participant.
Our Specialist Navigators are qualified Social Workers and draw on their professional knowledge to inform their work across complex systems.
Because Specialist Navigation is the only NDIS service WIRU provides, our focus remains on navigation rather than delivering direct care, accommodation or other disability services.
Our approach is strength-based, trauma-informed, culturally responsive and person-centred.
